| I hereby authorize Camp Gladiator, Inc., a Texas corporation, to automatically charge my credit card, debit card or bank account in the amount of $69.00 per camp on the Tuesday of the off-week for six (6) consecutive camps. Spouse and friend referral discounts will be applied where necessary. I understand the following: Camp Gladiator, Inc. is released from any other obligation to furnish me with any further notices of this payment agreement. I cannot cancel prior to completion of contract as I am receiving a significant reduction in the normal charges for Camp Gladiator, Inc. fitness camps. I have the ability to freeze my membership for only one (1) camp per ten (10) camps and understand that in order to redeem this freeze I must contact registration@campgladiator.com fifteen (15) days prior to the payment date and my account will not be frozen unless I have received confirmation of my eligibility and that it has been implemented. There is a $5.00 fee to freeze that will be applied as credit to the cost of the camp following the freeze. I am liable for the complete balance agreed upon with this contract. After the six (6) camp payments, my account will continue to be automatically drafted $69.00 per camp, on a per camp basis for the same amount, unless I send written notice of cancellation via mail or fax fifteen (15) days prior to the payment date to Camp Gladiator, Inc.
